How to Get Started

New patients can follow these four steps to set up portal access before their first visit.

Use the scheduling link to choose a date and time.

You will receive a confirmation email after scheduling.

Register and complete your intake forms before your visit.

Schedule Your Appointment

Click the link below to schedule your appointment.

While scheduling, the system may ask if you want to add a credit card on file for future billing.

  • If you want to add a card, fill in the information and click Submit to proceed to the next step.
  • If you do not want to add a card, leave the fields blank and click Submit to proceed to the next step.

After clicking Submit, you can then finish scheduling your appointment.

Check Your Email

After you schedule your appointment, you will get a confirmation email. If you did not receive an email, check your junk/spam folder.

You will use that same email address to create/register for your patient portal account.

Create Your Portal Account

Please create your portal account before your appointment.

** New patients cannot create a portal account until they have scheduled an appointment and received a confirmation email.  

Portal Login / Registration

In the portal you can:

  • Fill out your intake forms
  • Sign your visit consent forms
  • Upload your ID
  • Add your insurance information (if needed)

Forms and consents must be completed prior to your appointment. Your visit may be delayed if forms and consents are not finished ahead of time.

Use the Portal

Once your portal account is set up, you can:

  • Cancel or reschedule appointments
  • Message your provider
  • Add or update insurance information
  • Complete forms before your visit
  • Upload medical records
