Haven Health offers a concierge care option for patients seeking enhanced access, extended appointment time, and more direct communication with their care team. Concierge care is designed to support thoughtful, relationship-based wellness, gynecologic and reproductive healthcare within the scope of certified nurse midwives.

Monthly Membership
Concierge Care
$80 per patient, per month
Includes priority same-day or next-day scheduling, extended appointments, direct messaging with your provider, a comprehensive annual wellness plan, unlimited monthly visits, specialist care coordination, and personalized health resources.
What’s Not Included?
Hospital services
Emergency care
Surgery or anesthesia
Imaging or pathology fees
Services provided by outside clinicians
Medications or laboratory costs
Fertility services
Concierge care is not a substitute for emergency services.
Concierge Care Agreement
Enrollment in concierge care requires a signed Concierge Patient Agreement.
For transparency, patients are encouraged to review the agreement before enrolling.

Please note:
Reviewing or downloading the agreement does not activate concierge services. The agreement is not executed until it has been sent through Haven Health’s HIPAA-compliant patient portal and signed by both the patient and a Haven Health provider.
Concierge services begin only after all signatures are completed and confirmed.
